Personal Data and Why We Collect It
Consumer Information and HIPAA
In the operation of PDG’s behavioral health services, consumer information is collected and stored using a secure third party: Credible Behavioral Health Software. Read their privacy policy here.
All consumer information is strictly protected under HIPAA regulations and may only be used or shared for the sole purpose of providing services to that individual and with written consent from the individual. Website Form Submissions
Visitors to our website may have the option of signing up for our newsletter, commenting on posts, or submitting a message. In doing this, we may collect the following:
- First and last name
- Email address
- Phone number
- Information disclosed in the message or comment itself
- IP address, browser type, and referrer URL
These submission forms are optional and by clicking submit, the visitor is giving PDG permission to use and store this information. We do not sell this information to anyone. It is used only for coordination of direct care services, general announcements, and/or promotions and marketing. These forms are created and operated through the plugin Gravity Forms. Our Stay Connected form signs the visitor up for our regular newsletters, sent through MailChimp. At the bottom of each newsletter, there is an unsubscribe option that will permanently take the individual off the mailing list. We may still retain the initial contact information for the sake of future communication. Read the MailChimp privacy policy here.
Our Contact Us page transmits the visitor’s information to our WordPress dashboard. This information is then forwarded, as necessary, to the relevant party capable of answering the query or following up with the individual. Read the WordPress privacy policy here.
By submitting a comment to a post, the visitor is agreeing to post their provided information and comment publicly to the site. PDG reserves the right to not approve inappropriate or spam comments, in which case they will not appear on the site. We use the Akismet plugin to detect spam comments. Shop Transactions
PDG sells event tickets and merchandise online. In order to facilitate these purchases, PDG collects transactional data. This may include the following:
- Name, email address, and phone number in order to communicate with you regarding your purchase.
- Credit card/payment information in order to process the purchase.
- Username and password in order to create an account and save your checkout information.
- Products you’ve viewed in order to show you recently viewed item(s).
- Location, IP address, browser type, and shipping address in order to estimate taxes, estimate shipping, and send you your items.
This information is accessible by PDG executive management and website technicians only. Additionally, we use cookies to track your cart contents. Read below for more information on cookies.
In addition to the above provided reasons for collecting this data, we also collect and store it for the following purposes:
- Preventing fraud
- Complying with legal regulations
- Providing the best online transaction experience for you
- Legal, tax, and accounting records

Cookies
Cookies are small bits of data generated by a website and stored with your browser. They can serve many purposes, but they’re most commonly used for saving your login information from previous submissions or website visits. Every time you select “remember me on this computer,” you’re choosing to enable cookies for a set period of time so you have easier access.
In leaving comments or submitting forms through PDG’s website, you may choose to save your name, email address, and URL in cookies. These cookies are for your convenience and will last one year. In addition, PDG’s online shop uses cookies to keep track of your cart contents. You may also choose to save your checkout information for the next time you purchase something from the store.
Choosing to save your information for next time is always optional. Every browser has a way to wipe all current cookies available in its settings.
Analytics
Web analytics is the process of collecting site usage information in order to optimize the web navigation experience. PDG uses Google Analytics and Google Search Console to collect and view information about our users in order to understand how people use our site and how we can make it better.
Google Analytics collects data such as IP address, location, URL referral, type of browser or device, and more. Google Analytics also uses cookies sometimes to collect this information. What We Do With Your Data
PDG never, under any circumstances, sells or exchanges any data collected from you. PDG collects and uses all data for the sake of direct care services, communication, promotions, events, marketing, or analytics for PDG itself. PDG keeps this data indefinitely.
Basically, we’re trying to provide excellent services and continue to make PDG better for our consumers and the community. Please see above to learn about the specific data that’s collected and the reasons why we do so.
PDG uses several third party software programs and plugins to better manage data and operations. Each organization’s privacy policy is linked above in the relevant section.
In addition to those listed, PDG staff also extensively use G Suite, Google’s bundle of applications for business (such as Google Drive or Docs), in order to provide effective services to our consumers. G Suite is not accessible via PDG’s website. All information used in G Suite is protected by HIPAA and PDG has stringent privacy regulations in place to safeguard all consumer information.
